The Avantect Pancreatic Cancer Test kits can be requested by healthcare professionals who have a provider account with ClearNote Health. If you are a healthcare professional interested in establishing a provider account, please follow the prompts below to fill out the provider account set-up form. If you are a patient interested in information about this test, please contact your primary care physician.
Once a healthcare professional submits the form below, our Customer Success Representative will review your information and will contact you to confirm your provider account and explain the process for requesting a test kit and subsequently ordering a test.
